INSURANCE PLANS I take the following insurance and will verify your benefits and estimated co-pays prior to your first appointment:

FEES & POLICIES Co-pays will be determined by your insurance company and I will let you know the estimated amount before your first appointment. If you are out of network, I can provide a superbill for you to submit to your insurance company. I also accept clients who are self-pay. I require new clients to complete all intake paperwork electronically before scheduling an appointment and require a credit card to remain on file. If intake paperwork is not complete within 2 weeks, your slot will be given to the next person on my waitlist. Full payment, including co-pays, are due at the time of service. If you cancel with less than 24 hour notice or no-show, your card will automatically be billed $40. I take credit card, including HSA/FSA or cash. I close out new clients that no show the first appointment.
I also offer online counseling. This means we can meet over the internet, using the platform Doxy.me, which is similar to Skype or FaceTime. This software is free to you. Some insurance companies will cover the cost of online therapy and I can verify this benefit for you. If they do not, you will be responsible for my full fee, if you opt for online.
I will not complete paperwork for long term or short term disability until I have seen a client a minimum of 4 sessions. You will be expected to continue ongoing care after paperwork is completed.
