I do not take any EAPs (Employee Assistance Programs)
If using in-network insurance, your cost share will be dependent upon your plan's contracted rates and if you have a deductible. Please call your insurance for details.
*Prime members will need a referral from their PCM
**I am out of network with UGS but am sometimes able to do single case agreements with them
***You must have an approved authorization from the VA and coordinate with them to obtain
SELF-PAY FEES (no in network insurance):
Intake (your 1st appt) or 90 minute individual session: $225
45-55 minute session: $150
