INSURANCE PLANS I take the following insurance and will verify your benefits and estimated co-pays prior to your first appointment:
Aetna (in the process of credentialing) Anthem BCBS TriCare (I will be going to Certified Out of Network January 31, 2021. Prime members will need a referral from their PCM.) United Healthcare (UHC, Optum) / GEHA VA Community Care* *You must have a referral from the VA
FEES & POLICIES Co-pays will be determined by your insurance company and I will let you know the estimated amount before your first appointment. If you are out of network, I can provide a superbill for you to submit to your insurance company. I also accept clients who are self-pay. I require new clients to complete all intake paperwork electronically before scheduling an appointment and require a credit card to remain on file. If intake paperwork is not complete within 2 weeks, your slot will be given to the next person on my waitlist. Full payment, including co-pays, are due at the time of service. If you cancel with less than 24 hour notice or no-show, your card will automatically be billed $40. I take credit card, including HSA/FSA or check. I close out new clients that no show the first appointment.
I will not complete paperwork for long term or short term disability until I have seen a client a minimum of 4 sessions. You will be expected to continue ongoing care after paperwork is completed.
